1 · What the work actually is

Medical transcription in 2026 is mostly editor work: reviewing AI-generated drafts of physician dictation for clinical accuracy. From-scratch typing still exists but has shrunk to specialized dictation (heavy-accent speakers, noisy clinical environments, specialty terminology the model gets wrong).

Document types you will actually produce

• History & physical (H&P). Admission documentation with patient history, medications, exam findings. Highly structured.
• Operative reports. Surgeon's dictation of a procedure — anatomy, technique, findings, estimated blood loss. Specialty terminology dense.
• Consult notes. Specialist's response to a referral. Shorter but medically precise.
• Radiology reports. Imaging findings dictated by radiologists. Highly specialized vocabulary (spiculated, enhancement pattern, ground-glass opacities).
• Discharge summaries. End-of-stay summary combining all documentation. Longest and most error-prone for AI drafting.

Why it pays more than general transcription

A single wrong medication dose, misread lab value, or inverted anatomy in a medical transcript is a patient-safety issue. That risk floor is why agencies require credentialing, HIPAA training, and repeated quality audits — and why the per-audio-hour rate sits well above general-audio work.

2 · Pay rates and certification requirements

Medical transcription pay is tightly coupled to credentialing. Ranges below pull from AHDI-cited industry norms and each agency's posted tier (checked 2026-04-24).

The AHDI credentials

• RHDS (Registered Healthcare Documentation Specialist). Entry-level credential from the Association for Healthcare Documentation Integrity. Eligible: candidates with less than two years of acute-care experience, typically after completing an AHDI-approved training program. Three-hour exam. • CHDS (Certified Healthcare Documentation Specialist). Senior credential. Requires two years of acute-care documentation experience. Two three-hour exams (Part 1 written, Part 2 practical). CHDS is what hospital-system in-house supervisors and senior agency editors carry.
• Recertification. Both credentials require continuing-education credits every three years — typically 30 CECs for RHDS, 30 for CHDS.

HIPAA and privacy requirements

Every medical-transcription employer requires HIPAA training (free online; employers usually provide a signed acknowledgment workflow). You will be handling Protected Health Information (PHI). Agencies run background checks; in-house hospital roles additionally fingerprint.

3 · Where to find medical transcription jobs

Three paths: national agencies (highest volume), hospital-system in-house (highest stability + benefits), and marketplaces (entry if you accept non-PHI medical content).
