Medical transcription: how to find work, what it pays, what you need.

Medical transcription has consolidated around three paths: hospital-system in-house editor roles, large national agencies (Aquity, Nuance), and AHDI-credentialed freelance work. Physician dictation volume is still growing as EHR back-end audio capture expands — but AI-assisted editor roles are now the dominant job shape, not from-scratch typing.

$25–$60 per audio hour (RHDS tier) AHDI RHDS preferred credential HIPAA training non-negotiable

1 · What the work actually is

Medical transcription in 2026 is mostly editor work: reviewing AI-generated drafts of physician dictation for clinical accuracy. From-scratch typing still exists but has shrunk to specialized dictation (heavy-accent speakers, noisy clinical environments, specialty terminology the model gets wrong).

Why it pays more than general transcription

A single wrong medication dose, misread lab value, or inverted anatomy in a medical transcript is a patient-safety issue. That risk floor is why agencies require credentialing, HIPAA training, and repeated quality audits — and why the per-audio-hour rate sits well above general-audio work.

Medical transcription credentialing and pay path Four-stage workflow: medical-terminology course, RHDS credential, acute-care experience, then CHDS credential, with pay per audio hour labelled at each stage. From zero to CHDS: the medical transcription career path 1 · Terminology AHDI-approved program ~6–12 months No pay yet 2 · RHDS exam AHDI entry credential (3-hour exam) Agencies hire $25–$40 / audio-hr 3 · Acute-care experience (2+ years) Higher queues $35–$50 / audio-hr 4 · CHDS exam Senior credential (2 × 3-hour exams) Supervisor roles $45–$60+ / audio-hr Pay ranges synthesize AHDI-cited industry norms and agency-posted tiers (ahdionline.org, checked 2026-04-24). Real effective hourly depends on your audio-hour-to-working-hour ratio.
RHDS is the unlock from "trainee" to "hired." CHDS is the unlock from "hired" to "supervisor and senior-queue transcriber."

2 · Pay rates and certification requirements

Medical transcription pay is tightly coupled to credentialing. Ranges below pull from AHDI-cited industry norms and each agency's posted tier (checked 2026-04-24).

$25–$40Per audio hour · RHDS-credentialed transcriber at a national agency

HIPAA and privacy requirements

Every medical-transcription employer requires HIPAA training (free online; employers usually provide a signed acknowledgment workflow). You will be handling Protected Health Information (PHI). Agencies run background checks; in-house hospital roles additionally fingerprint.
